Ketamine for Chronic Pain: When the Nervous System Needs a Different Approach

Chronic pain is more than a symptom that lasts a long time. Over time, persistent pain can affect sleep, mood, movement, energy, relationships, and the ability to participate fully in daily life. In some people, the nervous system itself becomes increasingly sensitive to pain signals. This is one reason chronic pain may continue even after conventional treatments have provided only partial relief.

For selected patients with treatment-resistant pain, IV ketamine offers a different therapeutic approach. Rather than simply treating pain at the site where it is felt, ketamine acts on pathways within the central nervous system that help regulate how pain signals are processed and amplified.

Chronic pain and central sensitization

One important concept in persistent pain is central sensitization. With ongoing pain signaling, the brain and spinal cord can become more responsive to incoming stimuli — almost as if the nervous system's “volume control” has been turned up.

Ketamine primarily blocks NMDA receptors, which are involved in pain transmission, sensitization, and neuroplasticity. At sub-anesthetic doses used in pain medicine, this mechanism may help interrupt abnormal pain amplification and reduce certain forms of neuropathic and centrally mediated pain.

This mechanism is one reason ketamine is considered for some patients whose pain has not responded adequately to more conventional approaches.

Which chronic pain conditions may be considered?

Depending on the individual patient and clinical circumstances, medically supervised ketamine may be considered for treatment-resistant conditions such as:

  • Complex regional pain syndrome (CRPS)

  • Neuropathic or nerve-related pain

  • Fibromyalgia and other centrally sensitized pain syndromes

  • Certain refractory headache or migraine disorders

  • Other persistent pain conditions that have not responded adequately to standard treatment

Ketamine is not appropriate for every pain condition, and response varies. A careful medical evaluation is essential before treatment is considered.

Ketamine is not a stand-alone solution

Temporary perceptual changes, lightheadedness, nausea, or a dreamlike sensation can occur. Patients should not drive after an infusion and need a responsible adult to take them home. Treatment schedules are individualized according to the condition being treated, response, and clinical assessment.

The goal: better function, not simply a lower pain score

For people living with chronic pain, meaningful improvement may include more than a change in a number on a pain scale. Better sleep, increased mobility, improved mood, greater participation in family or work, and the ability to exercise or engage in rehabilitation can all matter.

Ketamine is not a cure for chronic pain, and it does not help everyone. But in appropriately selected patients, it may provide an opportunity to reduce pain amplification and create a window in which other restorative strategies become more achievable.
